Role Description

Join DaVita Integrated Kidney Care (IKC) as a Care Coordinator – Nurse Practitioner Primary Care Pilot. This role involves being at the forefront of healthcare innovation, where Nurse Practitioners (NPs) serve as independent, "primary care-lite" providers for complex CKD and ESKD patients.

  • Own the scheduling process for NP patient panels.
  • Assist in navigating patients through the continuum of care.
  • Act as the primary liaison between patients, the NP, and the partner nephrology office.
  • Participate in workflow optimization and refine processes.
  • Maintain accurate, real-time documentation in the Electronic Medical Record (EMR).
  • Identify and coordinate resources, referrals, and appointments.

Qualifications

  • High School Diploma or GED required.
  • 2+ years of experience in a professional office or clinical environment.
  • Proficiency in Microsoft Office (Excel, Outlook) and experience with Electronic Medical Records (EMR).
  • Excited about care redesign and comfortable with ambiguity and growth.
  • Excellent communication skills with clarity, empathy, and professionalism.
  • Self-starter who works well remotely and values communication within a team.
  • Ability to handle confidential patient information with integrity.

Preferred Qualifications

  • Genuine interest in nephrology, dialysis, or primary care.
  • 1–3 years of experience in a medical office, particularly in scheduling, referrals, or care management.
  • CNA, MA, or similar clinical background is a major plus.
