Qualifications
- A Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field (Master’s degree preferred)
- 2–4 years of hands-on experience in care coordination or case management, ideally with wound-care or chronic-care patients
- An active RN license required for nursing professionals
- CCM certification or eligibility is a plus

- Strong experience with EMR/case-management software
- Good grasp of healthcare and insurance systems
- Ability to work effectively with multidisciplinary teams
- Strong communication skills and excellent organization
- Ability to keep things running smoothly
