Key Responsibilities
Knowledge of Medicare, Medicaid, insurance and waiver programs and community resources.
Good problem-solving skills.
Integrity and self-discipline with the ability to work independently withing guidelines.
Understands and complies with CareStar Policies and Procedures.
Maintains confidentiality as related to patient information.
Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
Adheres to the CareStar Rule in performance of job responsibilities.
Minimum Qualifications
Experience providing care management/coordination preferred.
Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience.
Certified Case Manager (CCM) required or obtained within one year of being in the position.
Experience in completing assessments, service plan development and monitoring.
Effective organizational and communication skills.
Experience providing care management/coordination preferred.
