Job Description
• Authorizing initial admitting and doing concurrent reviews for medical necessity
• Collaborating with facilities and out-patient providers
• Monitoring care to ensure treatment is appropriate and effective
• Engaging internal and external resources for care plan and discharge planning
Job Requirements
- Master’s degree in social work, counseling or related field or with independent or associate licensure
- Licensure in social work, professional counseling or marriage and family
- Proficiency level typically achieved with three years clinical experience
- Must have a working knowledge of care management, case management, hospital and community resources
- Critical thinking skills, problem-solving abilities, effective communication skills, and time management skills
