ACCOUNTABILITIES
• The Sr. Manager, Population Health Pharmacist is responsible for designing, implementing, and overseeing pharmaceutical programs within a population health management framework. This role involves collaborating with healthcare teams, analyzing population health data, and developing strategies to optimize medication usage and improve health outcomes for specific patient groups.
ROLE AND RESPONSIBILITIES:
• Design and develop pharmaceutical programs aligned with population health goals and healthcare policies.
• Collaborate with healthcare providers, including physicians, nurses, and other specialists, to optimize medication therapy plans for specific patient populations.
• Analyze population health data to identify medication-related trends, gaps in care, and opportunities for intervention.
• Develop protocols for medication management, adherence, and safety, ensuring best practices are followed.
• Create educational materials and initiatives to enhance patient understanding of medication usage, adherence, and health outcomes.
• Implement quality improvement initiatives related to pharmaceutical services within the population health framework.
• Ensure compliance with regulatory standards, laws, and accreditation requirements related to pharmaceutical services.
• Provide leadership to the pharmacy team, offering guidance, training, and support to enhance their understanding of population health principles and practices.
• Establish and monitor key performance indicators (KPIs) to evaluate the effectiveness of pharmaceutical programs and interventions.
MINIMUM QUALIFICATIONS AND REQUIREMENTS:
• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
• Current pharmacist licensure in applicable state or region.
• 5+ years of experience in pharmacy practice, with a focus on population health management preferred.
• Strong understanding of healthcare analytics and data interpretation.
• Excellent communication and collaboration skills to work effectively within interdisciplinary teams.
• Demonstrated leadership capabilities with a focus on program development and implementation.
