Role Description

As our Virtually Integrated Care RN, you will be a pivotal provider, empowering patients toward optimal health through comprehensive remote support and collaborative care coordination. You will be instrumental in bridging care gaps and enhancing engagement in a dynamic virtual environment.

  • Meticulously assess patient needs and develop personalized care plans
  • Provide direct clinical coaching and proactively monitor progress
  • Coordinate diverse services and leverage telehealth
  • Diligently track compliance, document interactions, and contribute to quality improvement
  • Collaborate virtually with physicians, nurses, and other health care team members
  • Conduct virtual assessments of the medical, physical, and psychological status of patients
  • Respond to routine, urgent, and emergent patient issues
  • Oversee the patient care plan to implement preventive measures for patient safety
  • Drive risk reduction through participation in root cause analysis (RCA’s) and quality surveillance
  • Compile and interpret large data sets of patient information
  • Effectively coordinate communication with members of the bedside interdisciplinary team
  • Work collaboratively with bedside nurses and other care team members
  • Collaborate with unit leadership and provide oversight of unit quality programs and metrics
  • Ensure evidence-based practices are being followed by team members
  • Support management to ensure compliance with internal policies and regulatory requirements
  • Assess services, patient outcomes, quality control, and family/patient satisfaction
  • Identify needs for changes in standards, programs, and services
  • Develop recommendations for consideration by unit and hospital leadership

Qualifications

  • Bachelors of Nursing (or must be actively in pursuit of a BSN degree)
  • Graduation from an accredited program
  • Three years of related acute care work experience, which includes one year of leadership experience
  • Registered Nurse: AZ, upon hire
  • Basic Life Support - CPR, upon hire

Preferred

  • Masters of Nursing, upon hire
