Care Manager Mentor, Full-Time-Salaried Opportunity
Community Care is hiring a Care Manager Mentor to join our team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule. It is based in Waukesha with telework currently being the norm.
Care Manager Mentor Job Responsibilities:
- Provides on-the-job training, orientation and feedback to new Care Managers related to their assigned caseloads and job tasks. Serves as an on-going resource to Care Managers.
- Regularly communicates with the direct supervisor information about the new employees’ progress, needs for improvement and/or educational training.
- Participates in efforts to enhance work experiences. Coaches staff to achieve the highest level of evidence-based practice. Leads and supports to enhance employees’ work experiences and retain talent.
- Responsible for all job specific responsibilities listed in the Care Manager job description for Family Care or Family Care Partnership, as appropriate.
Care Manager Mentor Job Requirements:
· Education: Bachelor’s Degree in Social Work, Psychology or Human Services and 18 months experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity OR a Masters’ Degree in Social Work, Psychology or Human Services.
· Certification: Long-term care functional screener certification is required
· Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Care Manager Mentor Job Schedule:
- Monday to Friday, 8:00 AM-4:30 PM
