Role Description

Are you an experienced home health clinician or looking to transition or supplement from outpatient, SNF, or hospice settings? We're here to support you in delivering patient-centered care with unparalleled flexibility and independence.

• Provide Skilled OT Care: Perform in-home assessments to address activities of daily living (ADLs), instrumental ADLs, and home safety.
• Develop Adaptive Strategies: Create personalized treatment plans focusing on energy conservation, use of adaptive equipment, and functional strengthening for daily tasks.
• Manage Your Visits: Schedule and conduct patient home visits based on your preferred caseload and availability.
• Collaborate for Coordinated Care: Work with a supportive team to ensure a holistic approach to each patient's plan of care.
• Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!)

Qualifications

• Valid California Occupational Therapist license
• CPR or BLS certification
• Driver’s license and auto insurance
• Physical + TB test (within the last year)
• Professional liability and business license (if applicable)
