Emplify Health by Gundersen Health System is seeking a part-time Social Services Liaison. This is your opportunity to be a part of a great organization that prioritizes patient care!
- Work 24 hours/week
- Monday-Friday, no weekends or holidays
- Work remotely
- *Must live in WI, MN or IA*
- Partner on special projects and duties to assist our patients and their discharge needs
Qualifications
- Bachelor's degree in Social Work or in a related field
- 1 year of previous experience in human services or health care
