Job Requirements
- 1-3 years Microsoft Office
- 3-5 years practice experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology Managed Care and Medicare experience
- Healthcare licensure and/or certifications (RN, LPN, MSW, LCSW, CDE etc) highly preferred
- 5 years practice experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology Managed Care and Medicare experience
- Previous telephonic coaching, education or case management experience
- Excellent computer skills including MS Office applications
- Independent worker
- Excellent time manager
- Excellent analytical and problem solving skills
- Effective communications, organizational, and interpersonal skills, Bilingual a plus
- Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations
- Experience in facilitating client positive behavioral change
- Background in motivational interviewing, active listening skills.
