The role involves providing support for long-term services and supports (LTSS) care management, coordinating integrated care delivery for high-need members, and ensuring progress toward desired outcomes while maintaining cost-effective care. Essential duties include completing comprehensive member assessments, developing and implementing care plans, authorizing waiver services, and facilitating interdisciplinary care team meetings.

The LPN Care Coordinator manages and coordinates care for enrolled patients by developing, implementing, and monitoring individualized care plans in collaboration with clinical teams. Responsibilities include conducting monthly care calls, educating patients on chronic conditions, assessing medical conditions, and linking patients to community resources.
