The Telephonic Nurse Case Manager II is responsible for care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. Duties are performed telephonically or on-site, such as at hospitals for discharge planning.

The RN Case Manager will partner with a Care Coach to lead health-related needs, conduct regular health assessments, and develop individualized care plans for members in the Wisconsin Family Care program. This includes coordinating with outside providers for services like medication administration and skilled nursing visits to ensure comprehensive care.

This role involves overseeing daily administrative operations, supporting leadership, and ensuring smooth coordination across the CFR function by leading a team of remote staff to meet customer service goals. Responsibilities include hiring, training, developing staff, auditing performance, managing day-to-day needs, and ensuring team performance meets company standards.

This role is directly responsible for achieving window and door unit volume growth across assigned regions by partnering with retail management to implement localized strategic initiatives. Key activities include advising on organizational design, talent reviews, logistics, and enforcing legal and brand standard compliance.
