Role Description

This is a remote, full-time/benefits eligible position of 30 hours per week (.75 FTE). The position is field-based out of Southern Wisconsin.

The Wisconsin Women’s Health Foundation (WWHF) is seeking a professional to join our First Breath team as a Maternal & Child Health Educator. First Breath is a statewide program that helps people make positive changes to their tobacco, alcohol, and other substance use during pregnancy and beyond. The Maternal & Child Health Educator will provide evidence-based tobacco treatment, substance use, and mental health early intervention services to pregnant and postpartum individuals residing in Wisconsin.

Responsibilities

  • Health Education, Counseling, and Case Management (80%)
    • Manage a caseload of pregnant and postpartum individuals residing in Wisconsin.
    • Provide intensive, individualized tobacco treatment/smoking cessation counseling and relapse prevention counseling to participants via virtual visits, phone calls, and text messages.
    • Provide mental health education and referral support.
    • Provide virtual education to family members with the goal of reducing infant and childhood exposure to Environmental Tobacco Smoke.
    • Perform formal screening, brief intervention, and referrals for substance use.
    • Perform formal screening, education, and referrals for mental health.
    • Conduct screening, referrals, and follow-up for other health and social service needs.
    • Documentation and online data entry of all services on a daily and weekly basis.
  • Community Outreach & Engagement (10%)
    • Organize and facilitate annual Participant Advisory Groups and other participant engagement activities as determined by the annual workplan, funders, and/or supervisor.
    • Develop and maintain relationships with health and social service providers in Wisconsin.
    • Represent the First Breath program and serve as an advocate for participants at local community outreach events and on coalitions and workgroups.
  • Program Communication, Operations, and Development (10%)
    • Attend and contribute to monthly WWHF agency-wide staff meetings.
    • Attend and contribute to regular First Breath team meetings, including preparing case briefings; preparing and facilitating presentations; and participating in group discussions.
    • Engage in regular one-on-one coaching sessions with First Breath Program Manager.
    • Engage in regular quality assurance activities.
    • Respond in a timely manner to all requests for information from First Breath Program Manager and other WWHF staff.
    • Provide feedback and recommendations for program development and quality improvement.
    • Assist in the development of new counseling tools and educational materials.
    • Ensure the unique needs of women of Wisconsin are represented in perinatal programming at the Wisconsin Women’s Health Foundation.
    • Engage in ongoing continuing education opportunities and share learned knowledge with the team and organization.

Qualifications

  • Knowledgeable about maternal and child health, tobacco, and/or substance use.
  • Knowledge and experience working in mental health.
  • Experience working with diverse communities and demonstrated cultural humility.
  • Demonstrated ability to handle protected health information.
  • Excellent interpersonal communication skills and ability to treat participants with dignity and respect.
  • Excellent organizational skills, attention to detail, and ability to multitask.
  • Ability to work independently and demonstrated self-starter.
  • Strong technology skills including experience with Microsoft Office, data entry software, and text messaging.

Requirements

  • Bachelor's degree in Psychology, Social Work, Health Education, Human Services, Nursing, or a related field. Master’s degree in Mental Health field preferred.
  • 5 years of experience in an area related to patient care, case management, health education, and/or counseling.
  • Valid driver’s license, reliable vehicle, and car insurance required.
  • Ability to travel occasionally throughout Wisconsin and to WWHF main office in Madison, Wisconsin (employee will be reimbursed for all work-related travel).
  • A fully secure and confidential workspace (computer, phone, printer, and other supplies are provided).
  • Ability to obtain and maintain Certified Tobacco Treatment Specialist (CTTS) within six months of hire, Wisconsin Mandated Reporter training, and other trainings and certifications as determined by supervisor.
  • Bilingual individuals strongly encouraged to apply.
