• Conduct virtual musculoskeletal triage and follow-up visits to assess symptoms, functional limitations, and clinical risk. • Apply strong clinical reasoning to determine appropriate clinical recommendations within a virtual care model. • Deliver evidence-based virtual physical therapy interventions focused on functional improvement and symptom management. • Develop individualized care plans aligned with patient goals, functional needs, and long-term outcomes. • Identify clinical needs requiring care coordination and route tasks to Care Coordinators. • Communicate and coordinate with patients, care coordinators, APPs, and external referral sources to support continuity across the episode of care. • Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory, payer, and organizational requirements.
