This position assists in developing, assessing, and coordinating holistic care management activities to achieve quality, cost-effective healthcare outcomes, which includes developing personalized service care plans for long-term care members. The coordinator evaluates member needs, identifies available resources, and facilitates plans while coordinating services between members, caregivers, and providers.

The Care Coordinator is responsible for evaluating member needs via the annual Health Risk Survey, addressing social determinants of health, and coordinating care across the continuum for Dual Eligible Special Needs Plan (DSNP) members. This involves collaborating with the care team, providers, and community organizations to close gaps in preventive and health maintenance care.

The Registered Nurse provides telephonic triage directional patient care advice for general and specific illnesses, health-related issues, counseling, advocacy, education, and resource management to patients and families. This role involves conducting health assessments, monitoring patient health remotely, aiding in emergencies, and collaborating with the interdisciplinary care team to achieve optimal patient outcomes.

The Clinical Specialist will provide focused clinical case coverage, training surgical teams on the safe and effective use of the medical device, and managing inventory needs within assigned accounts. This role also involves providing clinical support for physician and sales training programs.
