Social Worker (Work at Home – U.S.)

Position Summary

We are seeking a compassionate and dedicated Licensed Social Worker (LSW/MSW) to join our care management team. This is a 100% work‑from‑home position available anywhere within the United States.

Schedule: Monday–Friday, 8:00 AM–4:30 PM Your Time Zone
No weekends or holidays required.

In this role, you will collaborate closely with a registered nurse case manager using a holistic, member‑centered approach to assessment, care planning, and advocacy. You will support members across all stages of life by helping them navigate financial, transportation, safety, housing, and behavioral health resources. All member interactions occur telephonically or electronically.

This position plays a key role in empowering members to make informed medical and lifestyle decisions and works alongside a multidisciplinary clinical team to connect individuals with the right community supports.

Key Responsibilities

• Partner with RN case managers to assess member needs and develop individualized care plans.
• Research and connect members to community resources addressing social, financial, housing, and safety concerns.
• Support members through telephonic or electronic communication, promoting engagement and self‑advocacy.
• Collaborate with clinical team members to ensure coordinated medical, behavioral health, and social care support.
• Advocate on behalf of members to help them access services aligned with their benefit plan and health goals.

Required Qualifications

• Active, current, and unrestricted LSW, LMSW, LCSW, or LISW in state of residence.
• 2+ years of experience in medical social work or case management.
• Master’s degree in Social Work.
