• Provide clinical leadership, supervision, and operational oversight for a team of Behavioral Health Care Managers • Balance direct patient care, quality assurance, and cross-functional collaboration • Support onboarding and training of new Behavioral Health Care Managers • Monitor team performance against key CoCM metrics • Ensure team adherence to CoCM clinical workflows, documentation standards, and best practices • Identify trends, risks and opportunities for improvement across the patient population • Maintain a patient caseload, as needed, to support team coverage and model fidelity • Support mental and physical health of patients on an assigned caseload • Participate in regularly scheduled caseload consultation with the Psychiatric Consultant • Screen and assess patients for common mental health and substance abuse disorders • Provide education about common mental health and substance abuse disorders • Track treatment response and monitor patients for changes in clinical symptoms • Adjust treatment plans for patients who are not improving as expected • Facilitate referrals to evidence-based psychosocial treatments
