• Initiative diagnostic, preventive and rehabilitative procedures as appropriate to the patients care. • Ability to Create a Plan of Care by evaluation and assessment of patient’s condition and maintain continuity of and or multidisciplinary services whenever needed. • Participation in Case Conference meetings, and In-Service Reviews. • Timely Documentation according to state, federal and organizational guidelines.
