• Conduct initial assessments and create customized plans of care for each patient • Deliver evidence-based occupational therapy interventions for ADL/IADL limitations, home safety, fall risk, and cognitive function • Collaborate with physicians, nurses, physical therapists, speech therapists, and care coordinators • Provide family and caregiver training on therapy goals and safety strategies • Document evaluations, progress notes, and reassessments in the EMR system (Your Tailor Made Senior Services) • Update care plans regularly based on patient progress and changing needs • Comply with agency policies, Medicare regulations, and professional standards of care • Supervise Occupational Therapy Assistants (COTAs) as needed • Participate in interdisciplinary meetings and case conferences • Promote occupational therapy education in the community, if applicable
