• Perform virtual evaluations to determine older adults’ appropriateness for at-home VR-based speech therapy • Develop individualized plans of care targeting cognition, language, speech, voice, and functional communication • Provide ongoing clinical oversight, progress monitoring, and plan-of-care modifications using outcome data • Conduct follow-ups, re-evaluations, and discharges with functional, quality-of-life–driven goals • Document evaluations, progress notes, and discharge summaries in compliance with medical and facility standards • Coordinate care with physicians, caregivers, and interdisciplinary team members to support continuity of care
