Job Description
• Provide in-reach, engagement, education and support to adults with serious mental illness (SMI) or serious and persistent mental illness (SPMI) about community-based mental health services and supported housing options.
• Make minimum in-reach visits per day and meet weekly/monthly visit and Informed Decision-Making Tool completion requirements.
• Maintain caseload ensuring every person has a Peer Support Specialist visit prior to reaching 90 days; communicate with manager about events affecting minimum requirements.
• Coordinate face-to-face meetings with members/recipients and guardians to determine interest in integrated supported housing with tenancy rights and behavioral health services.
• Assess interest in supported housing, explain benefits and financial aspects, facilitate and accompany site visits to permanent housing, and review housing preferences.
• Explore and address concerns of members who decline or are ambivalent about supported housing; provide linkage to ongoing in-reach and education/exposure to housing options and supports.
• Educate members/recipients about services covered under the North Carolina State Plan for Medical Assistance, Medicaid 1915(b)(c) waiver, or State funded service array.
• Document all Peer Support Specialist activities, track outcomes and refusals, make documentation available to the State upon request.
• Participate in necessary NC state-level activities including training, information sessions, and committees related to Transitions to Community Living Initiative.
• Coordinate closely with Transition Coordinators, care management, and other Vaya departments to ensure smooth transitions and address barriers; perform other duties as assigned.
Job Requirements
- Must reside in North Carolina or within 40 miles of the North Carolina border; must live in or near Vance, Franklin, Granville, or Person County, NC.
- NC Certified Peer Support Specialist (Licensure/Certification Required).
- High school diploma or GED required.
- 1-2 years of experience working with the MH/SU population.
- Ability to access and use confidential healthcare information/PHI and comply with HIPAA and 42 CFR Part 2; must sign confidentiality statement.
- Ability to establish appropriate and respectful relationships with persons with disabilities and their families; ability to interact professionally with upset individuals.
- Knowledge of Medicaid, Special Assistance, Medicaid 1915(b)(c) waiver, and State funded service array.
- Ability to perform documentation and reporting of in-reach activities and outcomes.
- Ability to perform physical activities including crouching, reaching, walking, talking, hearing, repetitive motion; lifting up to 10 pounds; sitting for extended periods.
