The LPN Care Manager supports patients through telephonic outreach, care coordination, and ongoing health education. Manage and support patients’ healthcare needs via telephonic outreach Conduct and document visits by using several technology platforms and EHRs Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes as needed Facilitate patient access to appropriate medical and specialty providers Educate patients and family/caregiver(s) about relevant community resources Coordinate continuity of patient care with external healthcare organizations and facilities Support patient self-management of disease and behavior modification interventions Provide patient health counseling, education, and instruction Perform any other duties necessary to drive our values, fulfill our mission, and abide by our company values Use computer applications Make and answer phone calls the majority of the shift Welcome patients Review patient medical records Code and fill out insurance forms Schedule appointments Arrange for hospital admissions and laboratory services Handle correspondence, billing, and bookkeeping Escalate patient concerns or needs to appropriate provider
