Job Description
Role Description
The Program Coordinator for Population Health is responsible for population health programs that promote the delivery of patient-centered care.
- Educating stakeholders on program applications
- Collecting information and submitting required documents while protecting PHI
- Analyzing program needs and system care delivery practices to determine workflows, tools, and resources needed for success
- Assisting and training practice teams on program deliverables, timelines, and reporting
- Developing and maintaining training materials
- Analyzing and reviewing clinic performance with leaders and care team members monthly
- Coordinating workflow design by facilitating meetings with clinic teams, content experts, leaders, and IT to understand workflow gaps and improvement needs
- Tracking and completing workflow changes and participating in communication and training of such changes
Qualifications
- 3-5 years of health care experience
- Baccalaureate degree in nursing, health sciences, or business preferred
- Masters preferred
- Project management certification (preferred)
Requirements
- Experience with process improvement methodologies
- Experience with an electronic health record
- Team-building/facilitation of meetings
- Experience with change management
- Proficient use of Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, SharePoint)
- Development and implementation of training materials and presentations
- Lean or Six Sigma experience (preferred)
- Sedentary work - (prolonged periods of sitting and exert up to 10lbs force occasionally)
