The role involves ongoing supervision and management of the Community Health Worker (Promotora de Salud) team, including monitoring skills, administering performance reviews, and ensuring timely attendance and job function completion. Responsibilities also include implementing quality assurance measures, documenting activities, coordinating administrative support for community events, and facilitating case conferencing for team support.

Community Health Workers will engage with an assigned panel of patients to provide care navigation, appointment logistics, prescription support, and resource sharing while assessing and enrolling them in Social Determinants of Health programs. They will also collaborate with a multidisciplinary team, document interactions, and participate in community events to build trust and support patient activation.

This role owns the Care Operations product domain end-to-end, defining and driving the roadmap for mission-critical workflows in partnership with Engineering, Operations, and Clinical teams. Responsibilities include stewardship of existing operational tools while driving the transition to governed, instrumented workflows that replace fragmented processes.
