Role Description
The Cadence Health team is seeking a remote-based Nocturnal Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. In this role, you will:
- Lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure.
- Adhere to evidence-based guidelines for type 2 diabetes and hypertension.
- Oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider.
- Optimize treatment plans, adjust medications, and determine the appropriate level of care.
- Ensure timely, patient-centered, and scalable remote care.
Qualifications
- 5+ years experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high-performing medical center.
- Active multi-state compact RN license.
- ANCC or AANP certification required.
- Master’s Degree as a Nurse Practitioner with the willingness to expand state licensure as Cadence adds new markets and partners.
- Experience managing CHF with GDMT.
Requirements
- Active Compact Multi-State RN license.
- Active NP licensure in at least one of the following states: TX, MI, NY, NJ, PA, CA.
- Active NP licensure in two or more of the following states: TX, MI, NY, NJ, PA, CA, AL, CT, HI, TN, WA, IA, IL, AK, AR, OK, OR, MT, MS, RI, or WI.
- Licenses must be active and in good standing at time of hire.
- To ensure that our clinicians have the necessary tools for a successful remote work environment, home office setups must have consistently stable Wi-Fi with strong upload and download speeds.
- A Wi-Fi speed test is required before participating in the interview process to verify that these standards are met.
