Job Summary: 

CareLinx by Sharecare is looking for California licensed Registered Nurses to join our team. 

As an RN, you will be responsible for telephonically assessing patients for exacerbations of chronic diseases, collecting clinical history, and communicating with patients/families. 

Essential Job Functions: 

  • Interpret an electronic dashboard 

  • Call and communicate with patients and/or family members over the phone 

  • Collect clinical history 

  • Assess patients for exacerbations of chronic diseases such as, but not limited to: 

  • CHF 

  • COPD 

  • Diabetes 

  • Dehydration 

  • Others as needed 

  • Notify the following parties in the event of exacerbation: 

  • Patient’s PCP 

  • Third-Party 

  • Document assessments electronically 

  • Part-time hours, minimum of 28-hours per week 

 

Skills/Attributes: 

  • Compassionate and caring nature 

  • Demonstrates empathy and patience when assisting members with personal and sensitive care needs 

  • Provides support in a way that upholds dignity, comfort, and trust 

  • Reliable and professional - when you commit to a shift, you can be counted on! 

  • Strong verbal and written communication skills 

  • Able to work independently and manage varying client schedules 

  • Tech-savvy and comfortable with phone app-based scheduling 

Qualifications: 

  • Bilingual in English and Spanish preferred

  • Licensed Nurse in California, Utah, and Nevada 

  • Licensure in additional states preferred 

  • Experience in performing clinical history and clinical assessments in older, geriatric patients 

  • Minimum of 2 years of experience supporting the following fields: 

  • Acute Care Emergency Department 

  • Intensive Care Unit 

  • Telemetry 

  • Stepdown Unit/Progressive care unit 

Salary: $54/hour (rate based on client needs, shift type, and experience)
