Job DetailsJob Location: Carmel Hospice - Carmel, IN 46032Position Type: Full TimeOn-Call Registered Nurse (7 On / 7 Off)  After-hours support. Mostly remote. Critical impact.     The On-Call Registered Nurse provides after-hours triage, clinical guidance, and urgent in-person visits as needed for hospice patients receiving care in home or facility settings, supporting comfort, safety, and continuity of care.  Schedule, Hours & Territory  • Dedicated on-call role with a rotating 7 days on / 7 days off schedule  • Coverage hours are 4:30 PM to 8:30 AM  • Assigned coverage area determined by branch needs and patient census  What We Offer  • Predictable 7 on / 7 off schedule  • Mostly remote role with field support as needed  • Strong clinical and leadership support  Benefits  • Medical, dental, and vision coverage  • 401(k) with employer match at 100 percent on the first 3% and 50 percent on the next 2%  • Health Savings Account eligibility with employer contribution up to $500 for individual coverage and $1,000 for two-person or family coverage  • Company-paid Life and AD&D  • Voluntary disability, accident, critical illness, and hospital indemnity options  • Employee Assistance Program, WillPrep, and travel assistance resources  • Tuition reimbursement  • Paid time off and paid holidays  • Mileage reimbursement per company policy  Key Responsibilities  • Triage patient and caregiver calls after hours  • Provide clinical guidance and symptom management  • Complete urgent in-person visits as needed  • Coordinate care with interdisciplinary team members  • Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)  QualificationsPROFESSIONAL EXPERIENCE/QUALIFICATIONS: Help optimize the capital structure and support strategic initiatives. Maintains the agency’s mission, philosophy and core values. Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management. Always maintains patient confidentiality, including all HIPAA regulations. Regulatory Requirements Licensed or registered in the state in which he/she currently practices.. Must pass a criminal background check & MVR check. Completed health statement acknowledging ability to perform the duties of the position. Valid state driver’s license. Proof of current automobile insurance. Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred. TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).       Industry experience Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program. Active patient contact within past three years, preferred. Minimum 2 years of experience as a registered nurse, preferred     EDUCATION: Graduate from an accredited school of nursing Currently licensed as a registered nurse to practice in the state.
