• Develops, assesses, and facilitates complex care management activities for primarily physical needs members • Evaluates the needs of the member and recommends plan for the best outcome • Develops ongoing care plans and collaborates with providers • Identifies problems/barriers to care and provides appropriate care management interventions • Coordinates between the member and care provider team • Provides ongoing follow up and monitoring of member status • Provides resource support to members and care managers • Facilitates care management and collaborates with appropriate providers • Collects, documents, and maintains all member information • Provides education to members and their families on healthcare options and benefits
