JOB DESCRIPTION SUMMARY
Our LVNs are experienced in adult in-home health nursing. You will be responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. We ask our fulltime staff to participate in the on-call rotation monthly which might include evenings, weekends and holidays as needed. Prime Home Health can provide you with a professional, experienced, supportive administrative team.

QUALIFICATIONS
- Licensed Vocational Nurse (LVN) with current and unencumbered license to practice professional nursing in the State of Texas.
- Bachelor's degree in Nursing from an accredited program, preferred.
- Minimum One (1) year of adult in-home health care experience preferred.
- Excellent time management skills
- Bilingual (English/Spanish speaking) preferred.
- Possess and maintain valid CPR BLS certification.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
- Must have reliable transportation.
