The role involves interviewing applicants by phone to gather and assess information for administering benefits such as SNAP, Medicaid, and TANF, and performing eligibility determinations, redeterminations, and periodic reviews.
Physicians will provide high-quality preventative, acute, and chronic care in a fully remote, video-based setting, managing a dedicated panel of patients as their primary care provider. Responsibilities also include working on population health initiatives, referring patients when necessary, and supporting post-discharge care to reduce readmissions.
