Job Description
Role Description
The On-Call Registered Nurse provides after-hours triage, clinical guidance, and urgent in-person visits as needed for hospice patients receiving care in home or facility settings, supporting comfort, safety, and continuity of care.
Schedule, Hours & Territory
- Dedicated on-call role with a rotating 7 days on / 7 days off schedule
- Coverage hours are 4:30 PM to 8:30 AM
- Ingham County - Michigan

Key Responsibilities
- Triage patient and caregiver calls after hours
- Provide clinical guidance and symptom management
- Complete urgent in-person visits as needed
- Coordinate care with interdisciplinary team members
- Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)
Qualifications
- Help optimize the capital structure and support strategic initiatives
- Maintains the agency’s mission, philosophy and core values
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management
- Always maintains patient confidentiality, including all HIPAA regulations
Requirements
- Licensed or registered in the state in which he/she currently practices
- Must pass a criminal background check & MVR check
- Completed health statement acknowledging ability to perform the duties of the position
- Valid state driver’s license
- Proof of current automobile insurance
- Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred
- TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter)
Industry Experience
- Ability to work independently, make accurate, and at times, quick judgments
- Ability to supervise others appropriately
- Ability to respond appropriately to crisis outside of a hospital setting
- Acceptance of and adaptability to different social, racial, cultural and religious modes
- Completes Hospice training program
- Active patient contact within past three years, preferred
- Minimum 2 years of experience as a registered nurse, preferred
Education
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the state
