Job Description

This is a part-time field position that combines direct client care with oversight of caregiver performance to ensure quality and compliance with KalCare Health Care policies and state regulations.
- Conduct supervisory visits for personal care and companion clients as required by agency and payer standards.
- Perform skilled nursing visits as needed, including medication administration, wound care, and health assessments.
- Document all visits promptly and accurately in the EMR system.
- Communicate changes in client condition to the supervising RN or Administrator.
- Review and monitor caregiver performance during supervisory visits.
- Educate caregivers on client-specific needs and proper care techniques.
- Participate in client care conferences and team meetings as requested.
- Maintain compliance with all agency, state, and federal documentation standards.
- Part-time / Flexible scheduling based on client needs and caseload availability.
- Service Area: Metro Atlanta: Rockdale, Newton, Henry and Dekalb Counties
- Weekends not required
- Reports directly to the RN Case Manager and/or Administrator.

- Minimum 3 year of home care or skilled nursing experience preferred.
- Strong communication and documentation skills.
- Valid Georgia driver’s license and reliable transportation.
