Job Title: Partner Liaison & CHW Supervisor

Reports To: SCN Project Director


Social Care Network Summary: The New York State Department of Health has established Social Care Networks
(SCNs) as part of the 1115 Waiver Demonstration Amendment, The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners.



Job Summary: This position is responsible for providing centralized supervision and technical assistance to a team of Community Health Workers (CHWs) deployed as on-site resources for FLIPA Member organizations. The Community Health Worker Supervisor will also coordinate and collaborate with each CHWs on-site supervisor to ensure CHWs successfully identify, assess, refer, and monitor high-need individuals to ensure access to essential services while supporting providers and the Care Team through an integrated approach to care management and community outreach. This position is funded through March 2027.

Duties/Responsibilities:

Supervision of Community Health Workers

  • Support and guide Community Health Workers (CHW) who work in FLIPA Member Centers so they can successfully provide screening, navigation, and care management services.
  • Encourage a consistent, person-centered, and culturally responsive approach to all activities.
  • Create a positive learning environment where CHWs can grow their skills and improve over time.
  • Make sure CHWs complete all required training and participate in supervision, meetings, and other required activities.
  • Track CHW performance, Will establish and then help them meet goals for screening, navigation and care management.
  • Build strong working relationships with CHW supervisors at each Center to support staffing and problem-solving.
  • Share successes, challenges, and concerns with FLIPA leadership to strengthen partnerships with Centers.

CBO Partnership & Readiness Support

  • Build and maintain relationships with assigned CBOs as they prepare to join the SCN network.
  • Provide project management support—helping with workflows, resources, and implementation plans.
  • Offer technical assistance to help CBOs build their capacity for screening, navigation, and service delivery.
  • Support organizational change efforts so CBOs can successfully adopt SCN processes.
  • Monitor each CBO’s progress using dashboards and reports to ensure they meet key milestones.

Network Learning & Continuous Improvement

  • Participate in shared learning with Partner Liaison Specialists and the broader SCN team to stay aligned and improve processes.
  • Help develop and implement learning community strategies that promote knowledge sharing across the network.
  • Identify challenges, gaps, and opportunities for improvement in project implementation and relationship management.

Data, Reporting & Communication

  • Support data collection and reporting activities, including interpreting dashboards and communicating findings effectively.
  • Provide clear, timely, and transparent updates to leadership and partners regarding progress, needs, and emerging issues.

Cross Organizational Collaboration

  • Work effectively across multiple organizations, locations, and community settings to advance SCN goals.
  • Maintain strong communication channels to ensure coordinated and aligned project implementation.
  • Communicate CBO progress, needs, and risks to the SCN Project Director and FLIPA leadership.

Other Duties

  • Perform additional responsibilities as assigned to support the success of the SCN initiative.


Education & Experience:

  • Bachelor’s degree in human services, Social Work or other related degree preferred.
  • Equivalent experience in lieu of education may be considered.
  • Minimum of 1-3 years' human services experience.

  • Basic computer literacy, including the ability to use email, conduct online research, and create basic documents (MS Office Suite including Excel, Outlook and Word).

Compensation Range:

  • This range is derived from an annual salary equivalent of $65,000 to $85,000.

  • This position will be remote with periodic in-person meetings.
