The Partner Liaison Specialist supports approximately 8–10 Community-Based Organizations (CBOs) through the preparation and implementation of Social Care Network (SCN) activities, guiding them through assessments, planning, technical assistance, and operational readiness. This role functions as a relationship manager and project coordinator, ensuring each organization meets SCN requirements and succeeds within the network.

This role involves providing centralized supervision and technical assistance to Community Health Workers (CHWs) deployed at Member organizations, ensuring they successfully identify, assess, refer, and monitor high-need individuals for essential services. Additionally, the position supports CBO partnership readiness by assisting with workflows, resources, and implementation plans as CBOs prepare to join the SCN network.

This role involves providing centralized supervision and technical assistance to Community Health Workers (CHWs) deployed at Member organizations, ensuring they successfully identify, assess, refer, and monitor high-need individuals for essential services. The supervisor will also coordinate with on-site supervisors to integrate care management and community outreach efforts.
