Job Description

Job Title: Social Care Network (SCN) Partner Liaison Specialist

Reports To: SCN Project Director

Project Summary

The New York State Department of Health has established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment. Forward Leading IPA (FLIPA) serves as the SCN Lead for the Finger Lakes region, which includes 14 counties: Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates.

The purpose of the SCN is to strengthen the delivery of social care services for Medicaid members by coordinating efforts among community-based organizations (CBOs) and health care partners. Through shared data, technology, and collaborative practices, SCN aims to improve access to services, support timely and reliable referrals, reduce health disparities, and advance health equity across the region.

Organizational Overview

Forward Leading IPA (FLIPA) is a nonprofit association of safety-net providers dedicated to integrating primary care, behavioral health, and social care services for underserved populations across Upstate New York. Since 2017, the organization has partnered with FQHCs, behavioral health providers, and a rural health network representing eight county public health departments. FLIPA remains committed to community-centered care, collaboration, and promoting equitable health outcomes.

Job Summary

The Partner Liaison Specialist (PLS) is responsible for supporting approximately 8–10 CBOs at any given time as they prepare for and implement SCN activities. This role includes guiding organizations through assessments, planning, technical assistance, reporting, and operational readiness. The PLS serves as a relationship manager, project coordinator, and strategic support partner—helping each organization meet SCN requirements and succeed within the network. Position Type: Grant-funded through March 2027

Monitoring, Reporting & Communication

  • Monitor progress of onboarding CBOs using dashboards, reports, and milestone tracking tools.
  • Provide clear, consistent updates on CBO progress and needs to the SCN Project Director and FLIPA leadership.
  • Support data collection, interpretation, and communication of key findings across the project lifecycle.

Collaboration & Continuous Improvement

  • Participate in peer learning and group-based improvement strategies with fellow Partner Liaison Specialists and SCN team members.
  • Identify trends, challenges, and opportunities for process improvement across the CBO network.
  • Support the development and execution of learning community strategies to facilitate shared knowledge, standardization, and ongoing improvement.

Other Responsibilities

  • Work effectively across diverse organizations, geographies, and organizational cultures.
