SelectQuote, is a program that helps patients learn more about their chronic conditions and how they
can manage them outside regular office visits through a comprehensive treatment plan created by their
Care Navigator.
SelectPatient Management engages virtually with patients who have multiple chronic conditions such as
diabetes, hypertension, and obesity. Our services are critical components of primary care that promote
better health and reduce overall health care costs for the patient. The goal of this program is to improve
patient's health and well-being by reducing care gaps, developing lifestyle goals, connecting patients to
resources, and facilitating access to doctors to help manage their chronic conditions

$18/hour base pay plus monthly bonus opportunities based on productivity

About the Role
The Care Navigator provides ongoing proactive patient services through regular communication and personalized care plans for patients participating in the Chronic Care Management Program. They assess patient health status address concerns and facilitate access to community resources to improve overall well-being. Key responsibilities include patient education and support consistent patient check-ins detailed documentation and care coordination ensuring patients receive coordinated care across various healthcare providers such as PCPs specialists pharmacists and other healthcare services.
Supervisory Responsibilities:
- This position has no direct supervisory responsibilities.
Essential Duties and Responsibilities:
- Provide ongoing proactive services through monthly calls
- Review the patient's current health status and appropriately escalate concerns to the physician via the escalation protocols. (Follow the patient escalation protocols and workflows to ensure that the patient is receiving quality and standard care)
- Providing education related to medical conditions and factors impacting patient’s health
- Setting health-related goals based on medical conditions & patient’s priorities
- Identifying and assisting in closing clinical and non-clinical care gaps
- Empowering patients to make sustainable health behavior changes to improve their well-being
- Continually providing motivation encouragement and support
- Communicating with patient’s medical providers regarding important items such as new or worsening symptoms or medication access and adherence (per protocols)
- Sharing community resource options to help patients access the necessary resources at the right time (e.g. transportation food)
- Respond to patients providers and team leaders
- Create update and maintain care plans using SelectPatient Management's proprietary software
- Achieve monthly call goals based on Supervisor Care Navigator advisory on assigned patient caseload. Typically the expectation is CNs are making 40+ phone calls and/or 11+ patients billed per day
- Maintain productivity and quality metrics as defined
- Attend weekly team huddles to discuss organizational and team goals as well as strategies to best serve our patient population

Education and Experience:
- Certified or Registered Medical Assistant required
- 2 or more years of experience in healthcare or a related field (3-5 years preferred)
- Family practice internal medicine and geriatrics office experience preferred
- Ability to work full-time (40 hours per week)
Physical Requirements:
Work is performed indoors with potential for exposure to safety and health hazards related to office work. Could periodically travel to other office and operational sites. The noise level in the work environment is usually moderate.
