POSITION SUMMARY

The Reassessor is responsible for conducting in-home Medicaid Reassessments to determine continued eligibility and level of care for participants receiving in-home and community-based services. This position ensures compliance with Missouri Department of Health and Senior Services (DHSS) guidelines, accurately documents findings, and provides person-centered, professional interaction with clients, caregivers, and providers.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential duties and responsibilities include the following. Other duties may be assigned.

  • Conduct Medicaid Reassessments in clients’ homes in accordance with DHSS requirements and agency standards.
  • Evaluate clients’ medical, functional, and social needs to determine continued eligibility and level of care.
  • Document assessments clearly and accurately in the designated software systems within required timeframes.
  • Verify care plan tasks and time utilization, ensuring all services align with client needs and policy.
  • Communicate with providers, family members, and informal supports to gather collateral information as needed.
  • Maintain current knowledge of program regulations, policies, and procedures.
  • Participate in quality assurance reviews and corrective action processes.
  • Adhere to confidentiality, professionalism, and ethical standards in all interactions.
  • Perform other duties as assigned to support the goals of the Senior Services Department.

QUALIFICATIONS

REQUIRED EDUCATION and EXPERIENCE

  • Bachelor’s degree in any field required.
  • Strong assessment, interviewing, and documentation skills.
  • Ability to manage time efficiently and meet strict deadlines.
  • Excellent written and verbal communication.
  • Proficiency with computers, electronic case management systems, and Microsoft Office applications.
  • Strong interpersonal skills and the ability to work independently and as part of a team.
  • Must successfully complete Medicaid Reassessment Certification
  • Must possess a valid driver’s license and reliable transportation for home visits.
  • Must pass all required background screenings.

PREFERRED EDUCATION and EXPERIENCE

  • Prior experience working with older adults, individuals with disabilities, or in a social services, healthcare, or case management setting preferred.
