Position Description

Anoka County Community Social Services and Behavioral Health is committed to meeting people's needs through quality services. We are currently seeking multiple full‑time Certified Assessors to join our Long‑Term Services and Supports (LTSS) team, completing initial assessments for individuals seeking state plan and waiver services (DD, CADI, CAC, BI, AC/EW). Qualified candidates may be a Social Worker, Registered Nurse, or have a degree in a related field. LTSS provides intake and referral services to individuals seeking assessments related to long-term services and support. This position conducts in-person comprehensive assessments utilizing the MnCHOICES assessment tool to determine eligibility and the need for home and community-based services.

This is a full-time, exempt, hybrid position. Interviews will take place the week of April 6th, 2026, for those selected to move forward in the hiring process.

Pay & Benefits

  • Salary: $74,214 to $89,065 ($35.68 to $42.82 per hour).
  • Anoka County Salary Schedule Grade 34: $74,214 to $103,896 ($35.68 to $49.95 per hour).

Work Location

  • This position will work at the Anoka County Government Center, located at 2100 3rd Ave, Anoka, MN 55303.
  • This position may be eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in the office.
  • Expected work hours are Monday - Friday 8:00 a.m. - 4:30 p.m.

Job Duties and Responsibilities

These examples are not all inclusive and are intended to be illustrative of primary responsibilities of an Anoka County Anoka County Social Worker.

  • Conduct MnCHOICES assessment to determine eligibility for services including Personal Care Assistance (PCA) and MA Waiver programs: Alternative Care (AC) and Elderly Waiver (EW), Community Alternatives for Disabled Individuals (CADI), Community Alternatives for Care (CAC), Brain Injury (BI), and Developmental Disability (DD).
  • Assessor also assist individuals with applications for program funding and Rule 185-Developmental Disability Case Management.
  • Develop person-centered Support Plans.
  • Complete assessments of individual and family strengths and needs to develop, implement, and maintain service plans in accordance with respective laws, rules, and department policies.
  • Maintain complete, accurate, and up-to-date assessment records and effectively document case activity to support case decisions.
  • Assess and refers individuals, families, and children to appropriate community resources.
  • Conduct individual assessments at various locations including, but not limited to, homes and offices.
  • Coordinate and collaborate within the department, community, and other counties providers.
  • Advocate for individual's needs and resources within departmental framework.
  • Represent the unit, department, or division in committee assignments and interactions with community groups for service provision and program planning.
  • Respond to faxes, phone calls, in-person referrals and requests for resources related to LTSS.

Qualifications and Requirements:

Minimum Knowledge, Skills, and Abilities Needed

  • Bachelor's degree in Social Work, Nursing, or a closely related field, OR current licensure as a registered nurse.
  • Two years (2,080 annual hours) of job-related experience.
  • In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.Valid driver's license and vehicle available for business use.

Preferred Knowledge, Skills, and Abilities Needed

  • Master's degree or higher in Social Work, Nursing, or a closely related field.
  • Three years or more (2,080 annual hours) of job-related experience.
  • Experience in Disability, Elderly/Alternative Care waiver case management.
  • Experience in working with clients with developmental disabilities or related conditions.
  • Experience in Rule 185 case management.
  • MnCHOICES Certified Assessor.
  • Fluent in a language in addition to English.
  • Experience with state data systems such as Minnesota Medicaid Management Information System (MMIS), Minnesota AXIS (MAXIS) and Social Services Information System (SSIS).

Physical Demands and Work Conditions

  • Standard office environment.
  • Vision abilities required by this job include close vision, distance vision, and the ability to adjust focus, such as to work on computers. Ability to see colors, shades, and brightness.
  • Hearing abilities required for general and phone communication, signals, and machine sounds.
  • Assignments are sedentary, occasionally alternating between sitting, standing, walking, crouching, and kneeling as required to perform job responsibilities. Frequent wrist and finger manipulation to complete computer work.
  • Occasional lifting of 10-20 lbs.
  • Equipment used includes computers, phones, and standard office equipment.
  • Occasional travel to other county work sites as needed. Travel between work sites may require driving a county vehicle and/or a personal vehicle.
  • Reasonable accommodation may be made to enable qualified individuals with disabilities to perform essential job functions.
