About the Opportunity:
We are seeking an experienced and compassionate Tele-hospitalist Physician to join our growing digital health team. Incoming provider must be internal medicine or family medicine board eligible/certified, with experience in hospitalist medicine or critical care experience. In this role, you will provide broad-acuity inpatient care at home via telemedicine, while supporting a range of patients originating from multiple Wellstar hospitals. This position is ideal for a physician who is comfortable with virtual care delivery and has prior experience with hospital-at-home models or remote patient monitoring programs.
Candidates must reside in one of the following states to be eligible for this remote position: Alabama (AL), Florida (FL), Georgia (GA), North Carolina (NC), South Carolina (SC), Tennessee (TN), or Virginia (VA).
Key Responsibilities:
Provide general medical virtual care to acute care and post-discharge patients from all hospital services ranging from general medical, post-surgical, cancer, and OB patients (e.g., postpartum patients with elevated blood pressure)
Care at Home Tele-Hospitalist physician supports patient care ranging from acute care level needs to post-hospitalization support.
Utilize telemedicine to assess, diagnose, and manage hospitalized patients in their home daily.
Coordinate with the Brick-and-mortar physician teams on care plan for patients admitted into Care at Home programs.
Work with the Care at Home multidisciplinary care team to provide patient care using remote monitoring technology, in-home visits and virtual monitoring.
Document all encounters in the electronic medical record (EMR) in a timely and thorough manner
Participate in Quality Unit Council, team meetings, and daily huddles to support patient care.
