• Lead video and audio onboarding consultations with members at the start of their journey. • Deliver a welcoming, personalized introduction to the program to set each member up for success. • Capture and synthesize the information shared to enable a smooth, high-quality transition to the treatment PT. • Prepare clear handoff notes and collaborate with PTs to ensure continuity of care. • Help members overcome initial barriers, building confidence and readiness to begin treatment immediately after the call. • Contribute to team initiatives and cross-functional projects aligned with Sword’s strategic priorities.
