Schedule: Full-Time (Monday–Friday, 8-hour shift scheduled between 8:00 AM – 6:00 PM)
Pay Rate: $28 – $33 per hour

Overview

Chronic Care Staffing is seeking a compassionate and skilled Registered Dietitian licensed in South Carolina to join our fully remote Chronic Care Management team. In this role, you will provide virtual nutrition support and remote patient monitoring services to patients working toward improved chronic condition management and sustainable weight-loss goals.

This is an excellent opportunity for a dietitian who enjoys patient-facing coaching, working with health data, and making a direct impact through telehealth care.

Responsibilities

• Virtual Nutrition Coaching: Conduct engaging one-on-one nutrition coaching sessions with patients via telehealth
• Remote Patient Monitoring: Track, interpret, and respond to patient weight readings and other relevant health metrics
• Personalized Care Planning: Develop customized meal and lifestyle plans, including support for patients using medications such as Ozempic
• Patient Education: Provide guidance on nutrition, healthy habits, and long-term behavior change
• Care Team Collaboration: Partner with physicians, pharmacists, and healthcare staff to ensure coordinated care delivery
• Documentation & Reporting: Maintain accurate patient records in EMR systems and remote monitoring platforms
