Job Description
• Manage identified complex/catastrophic patients attributed to the organization
• Assess the healthcare needs of the enrollee
• Develop a comprehensive treatment plan
• Implement a treatment plan in collaboration with the PCP team
• Monitor and evaluate the effectiveness of the care plan
• Coordinate care among multiple healthcare providers and services
• Provide community resources to patients to avoid hospital admissions
Job Requirements
- Nursing diploma or Associate's degree in nursing
- 3 years of experience in a clinical setting
- Experience documenting in an electronic medical record
- Experience using Microsoft Office
- Experience working in a multi-disciplinary team environment
- Current Registered Nurse (RN) License in the state of practice
- Certification as a Case Manager (CCM) (preferred)
