Job Description
Role Description
The Referral Coordinator II supports referral services by remotely reviewing medical charts of patients referred to the Organ Procurement Organization (OPO) to evaluate donor suitability, monitor care progression, and provide accurate information for clinical and family care support in accordance with Lifebanc and regulatory agency policies and protocols.
- Maintain electronic health record (EHR) access for OPO service area facilities to perform remote chart reviews.
- Provide real-time remote review and reporting to the AOC and other designated staff, including relevant medical and social history, laboratory results, diagnostic interpretations, hospital plans of care, and potential family needs.
- Review new referrals to assist with prioritization of cases.
- Monitor active referrals for continued suitability and identify revised or changing plans of care to preserve donation opportunities.
- Perform remote analysis of referrals listed on the rounding board to support timely updates, case closure, or identification of the need for additional clinical or family support.
- Coordinate with Hospital Services team members and collaborate with Lifebanc and hospital staff regarding follow-up plans.
- Support OPO performance metrics through timely, accurate, and thorough remote case reviews.
- Demonstrate knowledge of medical and legal principles related to donor evaluation and authorization.
- Utilize Lifebanc and hospital profiles, policies, and procedures as resources and mentor coworkers and newer team members in their use.
- Maintain professional, accurate, and respectful communication with internal and external partners.
- Participate in Quality Assessment and Performance Improvement (QAPI) activities.
- Perform other duties as assigned and assist the AOC as needed, including screening new referrals, participating in discussions with hospital staff (RN) as an operational support to the AOC during periods of high volume or competing operational demands.

Qualifications
- Associates degree in a clinical healthcare field or related area required.
- Minimum of two years OPO experience or 2 years of critical care experience required.
- Referral Coordinator internal or external experience required.
- Background as an RN, LPN, Respiratory Therapist, Paramedic, EMT preferred.
Requirements
- Proven expertise in a related Lifebanc, OPO, or clinical position.
- Strong computer skills and electronic health record platform expertise.
- Advanced organizational, communication, interpersonal, and problem-solving skills.
- Ability to work independently in a remote environment.
- Ability to function effectively in a fast-paced, high-stress environment involving end-of-life care.
- Ability to prioritize workload and manage multiple cases simultaneously.
Physical Requirements
- Ability to lift 25 pounds and to sit or stand for extended periods of time.
- Remote position is supported within a strong team environment with periods of high stress and extended hours.
- On call rotation responsibility with 24-hr computer and cell phone accessibility.
