The Regional Care Manager will conduct outreach, intake, and comprehensive assessments for members, developing and implementing individualized care plans that coordinate medical, behavioral health, and social services. This includes providing in-person, telephonic, and virtual services while maintaining accurate documentation and collaborating with various stakeholders.

The role involves conducting outreach, intake, and comprehensive assessments for Enhanced Care Management (ECM) and Community Supports (CS) members, followed by developing and implementing individualized care plans. Managers will coordinate medical, behavioral health, and social services, including housing navigation and food access support, while ensuring timely documentation.
