The Care Coordinator will conduct over 50 outbound patient calls daily, serving as a key contact for high-risk patients enrolled in Medicare's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. Responsibilities include monitoring vital signs, conducting structured health assessments, and providing education on lifestyle choices to ensure patient compliance with care plans.
The Licensed Practical Nurse will create, update, and review patient care plans, conduct monthly telephonic check-ins to assess health status and medication adherence, and review Remote Patient Monitoring synced vital signs, escalating concerns as needed. This role also involves coordinating follow-ups, scheduling, and ensuring all documentation meets Medicare CCM and RPM compliance guidelines.
