This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.

Your Contribution:

• Create, update, and review patient care plans based on the provider’s plan of care
• Conduct monthly telephonic check-ins with patients to assess health status and medication adherence
• Review RPM synced vital signs and escalate concerns to the provider when appropriate
• Coordinate follow-ups, scheduling, and documentation updates
• Ensure all documentation meets Medicare CCM and RPM compliance guidelines
• Accurately document all patient interactions within multiple EHR systems

Your Qualification:

• Active LPN License (Multistate Nurse Compact License Required)
• Strong verbal and written communication skills
• Minimum 1 year in related Chronic Care Management services
• Experience working with EHR systems
• Ability to work independently with minimal supervision
• Reliable high-speed internet and a quiet, HIPAA-compliant workspace
