The Nurse Practitioner will conduct virtual clinical assessments for recently discharged and high-risk patients, managing care plans and completing medication reconciliation remotely. This role also involves providing real-time support to field clinicians conducting in-home visits to support readmission avoidance and care continuity.

The Manager, Proposal Development leads the creation, writing, and coordination of high-quality proposals, RFP responses, and grant submissions to support business growth. This role centrally coordinates proposal development by working cross-functionally to translate complex operational capabilities into clear, compelling written documents.
