Job Requirements
- Unrestricted RN license in Compact States and CA
- Case management experience is preferred
- Strong clinical background with 5+ years nursing experience
- Experience and/or certification in specialty areas, such as cardiology, renal disease, or diabetes, is desirable
- Strong interpersonal and communication skills
- Comfortable communicating with patients and caregivers, physicians, and other clinicians
- Critical thinking skills to address top-level concerns and leverage all available data to determine the best path forward
- Strong personal and professional judgement
- Ability to respond quickly to shifting realities and adjust initiatives and priorities accordingly
- Demonstrate leadership ability; experience leading teams, projects, and client implementations
- Capable of working independently and demonstrates self-motivation
- Excellent documentation skills
- Adheres to the CRMS policy; exudes confidence and positive body language
- Understanding of all HIPAA regulations and the need to strictly adhere to them
- Proficiency with MS Office Suite including managing MS SharePoint and Excel
- Proficiency with internal messaging platforms including Microsoft Teams
- Proficiency with virtual meeting platforms including Zoom, WebEx, Skype, and Microsoft Teams.
