Job Description

• Apply advanced skills to provide care management services to patients • Connect patients with healthcare providers and resources • Provide patient-centered and trauma-informed care • Collaborate with a team of physicians, clinicians, and RNs • Review lab results and diagnostic imaging/studies • Develop and evaluate care plans for high-risk patient cases • Discuss treatment plans and provide health education • Document accurately and thoroughly • Evaluate the effectiveness of care plans • Integrate equity and inclusion best practices into all job functions

Job Requirements

Registered Nurse licensure in Michigan
Two or more years of clinical case management, care management, and/or care coordination experience, preferably in an ambulatory/outpatient setting
Caring, professional demeanor and excellent customer service skills
Excellent critical thinking, assessment, and problem-solving skills
Ability to work independently with minimal supervision
Experience working in Electronic Medical Records, especially Epic
Ensure protection of patients’ privacy and Protected Health Information (PHI) in a home office environment
Reliable wi-fi.
