The Nurse Practitioner will serve as the primary after-hours clinical support, providing medical direction and care telephonically to residents in senior living communities and skilled nursing facilities. This involves managing acute changes, falls, and controlled substance visits while coordinating care across various facilities and providers.

The Care Manager will deliver telephonic care management for patients in Value-Based Care Programs, supporting quality and cost-effective care by reviewing records, developing care plans, and educating patients and caregivers. Key duties include supporting quality gap closure, scheduling provider visits for at-risk patients, and facilitating orders and referrals.

The role involves managing incoming patient calls professionally, greeting patients, and assisting with inquiries or directing them to the correct department. Essential duties include conducting thorough patient health assessments over the phone to determine urgency and guiding patients to suitable healthcare resources.

This role is responsible for ensuring a smooth onboarding experience for new patients by guiding them through registration, collecting necessary documentation, and scheduling initial visits telephonically. The coordinator will also manage patient records in the EHR, address billing and insurance questions, and handle general patient communications via phone and email.
